Service Description

You will be asked to complete a new patient form, which includes: your contact details, relevant medical history and current prescribed medication (please bring an up to date prescription if you are prescribed any medication from your GP). Your podiatrist can help you complete this form if you are unsure, and they will then complete all necessary assessments which will help to shape your personalised treatment plan. Your first treatment may also be included during your initial consultation.
